Open Door MedBank is designed for individuals (adults and children) who are uninsured or underinsured and in need of prescription medications for a chronic illness that are out of reach financially.

Self-Referral Form: If you or someone you know is struggling to afford prescription medications, Open Door Medbank may be able to help. We provide assistance in accessing free or low-cost medications through pharmaceutical assistance programs and other available resources.
To begin the self-referral process, please complete our online form. Once form has been submitted, an Open Door Medbank representative will review your information and guide you through the next steps to determine eligibility and connect you with the help you need.

Provider Referral Form: If you are a physician, nurse, social worker, case manager, or other healthcare professional, you can refer eligible patients to Open Door Medbank for assistance with obtaining free or low-cost prescription medications.
